Our Team

Dr. Dayana Eslava
Dr. Dayana Eslava is a board-certified cardiologist with 20 years of experience in comprehensive cardiovascular care. She is board certified in Cardiovascular Disease, Nuclear Cardiology, Echocardiography, Vascular Ultrasound Interpretation, and Lifestyle Medicine. Dr. Eslava earned her medical degree from El Bosque University in Bogotá, Colombia, and completed her training at Beth Israel Medical Center and Long Island Jewish Hospital in New York City. She is a native Spanish speaker​.

She served as Assistant Professor and Medical Director of Outpatient Cardiology at Mount Sinai Hospital for 12 years before founding Heart of NY Cardiology in the Bronx, with a mission to advance health equity and improve access to high-quality care.

Dr. Eslava provides compassionate, patient-centered care using an integrative approach that combines evidence-based cardiology with lifestyle medicine. Her expertise includes heart failure, coronary and valvular disease, hypertension, hyperlipidemia, arrhythmias, preventive cardiology and lifestyle medicine. She is affiliated with Northwell Health - Lenox Hill Hospital and Cardiovascular Institute.

Office Visit Copays, Coinsurance, and Deductibles

A copay is typically a fixed amount associated with an office visit and diagnostic services, depending on your health plan agreement. Copays are collected at the time of service.

Coinsurance is based on a percentage of the allowed amount for a covered service and varies depending on your healthcare plan. Coinsurance applies after the deductible has been met.

Deductibles are predetermined amounts and are the patient’s responsibility. Deductibles are assessed annually, based on your benefit plan level, and are collected at the time of service.
